What Is Supported Living?

Supported living means living in your own home, whether that is a flat, a shared house or a tenancy with a housing provider, with the care and support you need coming to you. It is built on the idea that everyone has the right to an ordinary life: their own front door, their own routine and real choice over how they spend their day.

Eternity Care provides the support side of that arrangement. Where personal care or medication support is needed, it is delivered under our CQC registration with full care plans, risk assessments and records. Alongside it, our support workers help with the practical things that make independent living work, from cooking and budgeting to appointments, work, learning and getting out into the community.

Who We Support

Adults with learning disabilities

Autistic adults

People living with mental health conditions

Adults with physical disabilities

People with acquired brain injury or neurological conditions

Young adults moving on from the family home or children's services

We support people in their own flats and houses, in shared supported living schemes and in tenancies arranged by local authorities and housing providers.

What Our Supported Living Support Includes

Personal care and medication: Delivered with dignity under our CQC registration, only where it is needed.

Daily living skills: Cooking, cleaning, laundry and keeping the home safe and comfortable.

Money and tenancy: Budgeting, bills, benefits and keeping the tenancy on track with the housing provider.

Health and appointments: GP, dentist, hospital and therapy appointments, and following health plans.

Work, learning and activities: Support to attend college, volunteering, work and the things the person enjoys.

Community and relationships: Getting out, keeping in touch with family and friends and being part of the local area.

Emotional and behavioural support: Calm, consistent support that follows positive behaviour support plans where they exist.

Every person has a support plan written with them, in their words, setting out what they want from their life and how we help them get there.
